MJ’S DOG TRAINING Policy Form 

A non refundable deposit of $75 is required to reserve your appointment.  

Cancelling with less then 24hrs of a session, or a no call/no show 

will result in a forfeiture of the deposit. 

You may reschedule your appointment with no penalty. An appointment may only be rescheduled one time.  

*Sessions expire 6 months after date of purchase. It is up to you to schedule.  

If you are coming to me:

I will only wait 15 minutes past your scheduled appointment time before considering it a no call/no show.  It is up to you to contact me to reschedule. 

If you show up late, your session will not be extended, as some times clients are scheduled back to back.  

Please show up on time and NO earlier than 10 mins prior to your appointment. 

There is no smoking allowed on the property. 

I do not have a public bathroom. Plan accordingly.
